Hospital-to-home and recovery care in Longmont, CO

Coming home is the risky part.

What it is, in our words

Coming home is the risky part. Hospital-to-home care starts with the discharge itself (pickup, belongings, mobility help, and settling in safely) and continues with the short-term, non-medical support that makes recovery go the way the doctor drew it up: meals, laundry, personal care, help while using a walker or crutches, rides to follow-ups, note-taking at appointments, and overnight companionship in the first nights. It covers outpatient procedures too: plastic or cosmetic surgery, dental surgery, LASIK, colonoscopy, and C-section or postpartum recovery for the recovering adult.

Who it is for

  • Someone being discharged from UCHealth Longs Peak, Longmont United, or a rehab center who cannot go home alone
  • A patient who needs a responsible adult for an outpatient procedure and does not want to ask a neighbor
  • A new mother recovering from a C-section who needs meals, laundry, and rest support for herself
  • A person recovering from a fall, stroke, or surgery who needs help for a few weeks, not forever

What we do

  • Hospital and rehabilitation discharge pickup, belongings and mobility help, and settling in safely
  • Outpatient procedure escorts and the paperwork facilities require
  • Meals, laundry, personal care, mobility help, errands, and rest support during recovery
  • Help while using a walker, wheelchair, crutches, brace, sling, or cast
  • Rides to follow-up visits, appointment companionship, note-taking, and reminders about written instructions
  • Overnight companionship and safety monitoring during recovery

What we do not do

  • Skilled nursing, medical diagnosis, or medical treatment
  • Injections or medication administration (we do medication reminders)
  • Wound or drain care, feeding tubes, or respiratory treatment
  • Physical, occupational, or speech therapy
  • Emergency or medical transportation (rides are in an ordinary passenger vehicle, within our non-medical scope)

  3. 3
A plan built around the person.A few hours, overnights, recovery support, or ongoing care. There is no required minimum number of care hours per day or week.
  4. 4
A caregiver matched by personality. We match personalities, not only schedules and care needs. You always have the office to call, and care can go up or down as needs change.

From a family we serve

I like the fact that they would sign the paperwork and assist me with getting out of the hospital. I would not have been able to get the surgery I needed without the agency.

Hospital-to-Home & Recovery Care: questions families ask

Can you pick someone up from the hospital or a procedure?

Yes. We provide discharge pickup and outpatient procedure escorts, including signing the responsible-adult paperwork the facility requires. Tell us the facility, date, pickup time, and whether the person can travel safely in a regular passenger vehicle.

Do you provide medical care after surgery?

No. We are non-medical. We do not do wound or drain care, injections, or medication administration, and we do not provide therapy. We handle everything around the medical care: meals, mobility, personal care, rides, reminders, and overnight presence.

How long does recovery care usually last?

Whatever the recovery needs: a single procedure day, the first week home, or a few weeks. There is no required minimum and no long-term commitment.

Can you help after a C-section?

Yes, as support for the recovering adult: meals, laundry, personal care, mobility help, errands, and rest. We do not provide newborn medical care or stand-alone childcare.

Can care start before discharge?

Yes, and it is the best way to do it. Call while the person is still in the hospital and we can do the consultation and have the plan ready for the day they come home.
