Who we are
This website is operated by SYNERGY HomeCare of Longmont (legal entity HomeCare Colorado LLC), 2919 17th Avenue, Suite 115, Longmont, CO 80503, an independently owned and operated SYNERGY HomeCare franchise. Questions: synergycares@synergyhomecare.com or (720) 204-5788.
What we collect
If you submit the request form, we collect the information you enter: name, phone, email, preferred contact method, best time to call, who needs care and your relationship, ZIP code, and any notes you add. If you call or text us, we collect your phone number and what you tell us. We ask you not to include medical details on the public form; we collect those securely, by phone or in person, during intake.
How we use it
To respond to your request, schedule a consultation, and provide care. Form submissions are delivered to our office through our website host (Netlify) and may be entered into AxisCare, the agency system we use to manage clients, caregivers, and schedules. We do not sell your information.
Calls and text messages
By providing your phone number and checking the consent box, you agree that we may call or text you about your request. Message and data rates may apply. Message frequency varies. Reply STOP to opt out of texts at any time and HELP for help. Consent is not a condition of receiving care.
Cookies and analytics
This site sets no advertising cookies. It does not currently run analytics; if we add measurement (for example, call or form tracking) we will update this page to say what is collected and why. The optional map on some pages is loaded from Google only when you click to show it, and Google’s privacy policy applies to that map.
Health information
We are a non-medical home care agency. Information you share with us during care is handled under our client agreement and our Notice of Privacy Practices, which we provide at intake.
Your choices
You can ask us what we hold about you, ask us to correct it, or ask us to delete it, subject to records we are required to keep. Email synergycares@synergyhomecare.com.
Website terms
Content on this site describes non-medical services and is for general information; it is not medical advice and does not create a client relationship. We do not guarantee a specific caregiver, continuity, an immediate start, or any health outcome; care depends on assessment and caregiver availability. Reviews are quoted from public platforms as written. Trademarks belong to SYNERGY HomeCare Franchising, LLC and are used under license. We may update these terms; the date above will change when we do.
